We are happy to report that the issues with the new requests page has been resolved.


A change to simplify ticket forms caused users to experience issues when creating new tickets using the Help Center ticket form. The change was reverted to resolve the issue. We have added additional testing to catch these type of issues in the future before deploying to production.
